Medicare's Order List Grew by 1,260 and 435 Providers Lost Authority
CMS rebuilds the Order and Referring file twice a week, and the only number anyone looks at is how long it got. Between the 09/17 and 09/21 builds it got longer by 1,260 NPIs. Underneath that, 196 providers came off it entirely and another 239 kept their row while a flag switched off, which is the half no presence check will catch.
Twice a week, CMS rebuilds the list of every practitioner allowed to order a wheelchair, a lab test, or a home health episode for a Medicare beneficiary, and publishes the whole thing as a flat file.
The build stamped 09/17 carried 2,036,203 distinct NPIs. The build stamped 09/21, posted Monday afternoon, carries 2,037,463.
Net gain of 1,260. Read the headline count and nothing happened.

One Net Number, Four Real Ones
CMS keeps every dated vintage in the data catalog, 343 of them going back to June 2023, so the two builds can be diffed directly.
Between them, 1,456 NPIs were added and 196 were removed.
Another 479 appear in both builds with a different flag string: 240 gained an ordering authority and 239 lost one. Nobody did both in this build.
So the four-day movement is 1,696 gains against 435 losses, and the number everyone quotes is the difference between them (chart below).
That ratio is why the headline count is useless for this. Across the full archive it has fallen build over build only 23 times in 342 transitions, and the file has added 268,790 names since June 2023.
The list essentially always grows. The losses ride underneath.
