Cigna Posted 111 Changes and 55 Had Already Taken Effect
Cigna's August policy update went live on the 15th. Half its dated changes were already in force, including both reimbursement policies, and one of them keys your contrast payments to a Medicare indicator that splits the same tracer down the middle.
If you bill Cigna, your August rules changed on August 1. The notice went up on the fifteenth.
That is a scheduling fact with a claims consequence, and this month it is measurable, because Cigna stamps nearly every line of its monthly policy update with that line's own effective date.
We pulled the August 2026 edition this weekend, after four consecutive runs where the URL returned a 404 and the July file sat there returning 200.
Thirty pages. 111 dated changes, under a header reading "Effective August 15, 2026 (unless otherwise noted)."
Quite a lot was otherwise noted.
The Notice Arrived After the Change
Fifty-five of the 111, 49.5%, carry an effective date earlier than the day the document posted. Another 32 take effect on that same day.
Which leaves 24, about 21.6%, with any forward runway at all (chart below).

The biggest single block is the 48 drug coverage policy rows effective August 1. By the time the document naming them appeared, those had been governing authorizations for two weeks.
Three items in the document do carry real notice, and they are worth naming because of what they are: Breast Reduction (0152), Extracorporeal Photopheresis (0320), and Laser Interstitial Thermal Therapy (0528), all stamped posted 8/15/2026 and effective 11/15/2026. Ninety-two days.
So the runway exists. It goes to the medical coverage policies, the ones that decide whether a procedure is covered at all.
The two policies that decide whether a line gets paid went the other way.
