Everything That Moved This Week Came From One Payer
Since Friday the 18th, CMS has published exactly one document to the Federal Register, issued no transmittals, and revised nothing in the coverage database for twelve days. UnitedHealthcare posted six provider notices in the same stretch, two of them rewriting how you verify coverage in 2027. Here is the whole week, with the October 1 clock still running.
If you run a payer-policy watchlist, the last five days will have looked like your scraper broke.
It didn't. Three CMS publishing feeds went quiet inside the same fortnight, and they stopped in sequence: the Medicare Coverage Database on September 11, the transmittal queue on September 18, the Federal Register docket on September 22 with a single notice that has nothing to do with you (chart below).

Meanwhile UnitedHealthcare posted six provider notices in six days. So the lopsidedness is the finding this week.

The One Thing CMS Filed
Federal Register document 2026-19366, published September 22, is a Paperwork Reduction Act 60-day notice. Document identifier CMS-10142, OMB control number 0938-0944, a revision of a currently approved collection.
The collection is the Bid Pricing Tool that Medicare Advantage organizations and Part D plans use to build their actuarial bids. CMS puts the burden at 460 respondents, 11,700 annual responses and 406,000 annual hours.
Comments close November 23.
Affected public, verbatim from the notice: "Private Sector, Business or other for profits, and Not for profits institutions." No provider, supplier or billing entity is a respondent.
This is the plan-side paperwork cycle, and it is the only thing CMS has put in the Federal Register since the DNV accreditation notice on the 17th.
Five Days Without an Instruction
The 2026 transmittals table tops out at September 18, four documents deep:
- R247SOMA and R248SOMA, the State Operations Manual appendices for critical access hospitals and hospitals, effective and implemented the same day they issued
- R13967OTN (CR 14429), the HIPAA EDI package, implementation October 5
- R13965CP (CR 14607), instructions for retrieving the January 2027 Opioid Treatment Program payment rates file, implementation January 4, 2027
We ran the first three on the 19th. R13965CP is the one nobody has written up, and it is worth exactly what it looks like: a file-retrieval instruction for a quarter that starts 103 days from today.
The last operational thing CMS said to your MAC was about January.
