UnitedHealthcare's August reimbursement bulletin files the month's most expensive item under Revised, and labels the row Reminder.
Reminders do not usually switch off payment.
This one does. Effective October 1, outpatient UB-04 claims from in-network facilities carrying any of 93 revenue codes must also carry a CPT or HCPCS code, or they are not eligible for reimbursement.
The Federal Register went quiet after Thursday's rules, and the week's work moved to the payer bulletins and the CMS manual system.
